How does it work?

When you call

You will speak directly to a dedicated customer service associate who will ask basic details to establish if your needs can be met by REACH Physical Therapy @ Home
If appropriate, an appointment will be made for the therapist to visit at a time convenient to you

Initial Visit

During the initial visit the physical therapist will take a full history of the condition and complete a physical examination.
Findings of the assessment will be fully discussed along with treatment options and likely treatment outcomes.
Treatment goals will be set in consultation with the client and a plan formulated which will include frequency and duration of treatment sessions and costs.
Therapy will be started once the customer is satisfied with the goals and treatment programs

Subsequent visits

Treatment plans may include active and passive exercise programmes, manual therapy, re-education of functional tasks, mobility practice and environmental assessments.
Goals, plans and outcomes will be reviewed at all treatment sessions and modified as appropriate
Equipment needs will be assessed as necessary and options for the provision of equipment discussed as appropriate.
